Jonathan Weimer is a board certified family nurse practitioner at Vivent Health. He provides primary care to patients living with HIV, and believes in the patient-centered model of care because it encourages patients to be active participants in their own health and wellness. Jonathan has a passion for prevention and also provides PrEP to HIV-negative patients.
