Thrive Health Connection and Vivent Health (pronounced Vy•vent) are joining together as one organization, partnering to provide comprehensive health care programs and social services for at risk individuals. While this merger means some things will change, we’ll still be providing all the support you valued from Thrive Health Connection and adding even greater access to HIV prevention, care and treatment services and advocacy for thousands of people in the greater Kansas City area. And there’s one thing that will never change: All services will be delivered with the high level of radical hospitality you’ve come to expect.

As a result of the merger with Vivent Health, we are excited to bring to the greater Kansas City area the only HIV Medical Home in America that is recognized by the Centers for Medicare and Medicaid. The Vivent Health HIV Medical Home provides patients and clients with comprehensive integrated primary, specialty, preventive care and treatment services all in a vibrant, safe and accessible part of Kansas City.

Together in a New Location

As a part of the Vivent Health and Thrive Health Connection merger, we’re also moving into a new and convenient location. This 16,000 square foot state-of-the-art facility is conveniently located in the Shops on Blue Parkway, served by bus route 47. We have our own dedicated, free parking lot in a multi-purpose shopping center. And all our services—case management, medical care, behavioral health, pharmacy, food pantry, prevention services, and more—will be available in our new location.

  • Thrive Health Connection is now Vivent Health. What does that mean for Thrive Health Connection and people living with or at-risk for HIV?

    • At Vivent Health, we are committed to providing integrated HIV and primary care in Kansas City via the HIV Medical Home model of care.
    • We will significantly expand programs and services to build healthier lives for people in the greater Kansas City, MO/KS area who are at risk for and living with HIV/AIDS by implementing the nationally renowned Vivent Health HIV Medical Home.
    • Our HIV Medical Home offers fully integrated health and social services, including medical, dental, and behavioral health, along with pharmacy, case management, and social support provided by a team of professionals dedicated to patient-centered care.
    • A new 16,000 square-foot state-of-the-art facility opening in November 2021 includes a medical clinic, pharmacy, and food pantry.
    • HIV prevention services are a cornerstone of Vivent Health’s comprehensive care, and they will be available in our new location.

    • As Kansas City’s original HIV/AIDS services provider, Thrive Health Connection has grown to serve 1 in 6 people living with HIV in the region. Over the years, it has advanced on multiple fronts, adding enhanced programs and services for people affected by and living with HIV/AIDS. The Vivent Health and Thrive Health Connection merger is a natural next step in growing programs and services to meet the need in our community.
    • In Kansas City, approximately 4,000 people are living with HIV /AIDS, a rate of 226.1 individuals for every 100,000 population. There are an additional 150 new HIV cases reported annually. Thrive Health Connection operates in zip code 64130, the second-highest HIV/AIDS populated area in Kansas City.
    • The Vivent Health Medical Home Model will offer an expanded level of integrated care for people living in the greater Kansas City Metropolitan area.
  • How was this decision made?

    • Thrive Health Connection has always operated through the lens of what is best for people impacted by HIV/AIDS in our community. Thrive Health Connection put mission first when examining the merger as a means of meeting the needs of patients.
    • The Vivent Health HIV Medical Home is a powerful way to fight and ultimately end the HIV/AIDS epidemic by ensuring people living with and at risk for HIV have access to integrated, patient-centered care.

    • Vivent Health is a national leader in HIV/AIDS prevention, care, and treatment and has the best health outcomes in the United States, helping people with HIV/AIDS live long, healthy lives.
    • Vivent Health operates the only HIV Medical Home in America recognized by the Centers for Medicare and Medicaid Services.
