Vivent Health now Accepting Appointments for Onsite Services

We’ve implemented changes in our facilities to help keep everyone safe.

Starting Monday, June 15, 2020, Vivent Health is accepting appointment requests from current and new patients and clients for all services, except for dental care. Individuals can call Vivent Health at (800) 359-9272 or use their MyChart mobile app to make appointments at all of our locations.
 
Please note that you MUST have an appointment to enter the building – no walk-in appointments or visits will be allowed.

When arriving for visits, patients and clients will be required to wear face coverings, and should bring their own, if possible. Before entering any Vivent Health location, everyone will be screened for Covid-19.

Vivent Health is committed to protecting the health and safety of its patients, clients and staff. Everyone will also be using personal protective equipment, including face coverings. Plexiglass barriers are being installed as needed and hand sanitizer stations will be located throughout Vivent Health locations. ​We are also able to provide many of our services virtually.
