1. To request a medical appointment as a new patient...
First, click “request appointment” under the “New & Existing Patients” tab.

2. Sign up is easy!
When you click “request an appointment”, you’ll be taken into MyChart. Click on “Sign up now” on the right hand side of the screen.

3. No code, no problem.
You don’t need an activation code to sign up. Simply select “Sign up online” on the right hand side under “No Activation Code?” and proceed with the account creation process.
