Welcomed into the medical home

A patient came to us last year after a positive HIV test following an extended period of feeling ill. During his first Vivent Health appointment, he was seen by a case manager, a medical professional and had labs drawn. The patient was started on medications that same day and was immediately connected with the Vivent Health Pharmacy. Because he was struggling with his new diagnosis, and was facing complications with family, the patient was referred to Behavioral Health. We are pleased to report that the patient has kept all his appointments since coming into care with Vivent Health and as achieved an undetectable viral load.
