Medical

People living with HIV are at the center of everything we do at Vivent Health + TPAN, ensuring both your HIV care and general healthcare needs are met.

Because our unique HIV care model fully integrates social services and HIV primary care, our patients’ viral suppression rate is 95%, well above national averages.

Pharmacy

Vivent Health + TPAN Pharmacy is a full-service HIV pharmacy. We help patients with access to all their medications, not just their HIV medications. We also strive to minimize out of pocket cost for all patients. If a patient has concerns about being able to pay for their medications, we will help review their eligibility for assistance programs.

Vivent pharmacies are recognized by the Accreditation Commission for Health Care for Speciality Pharmacy Services with a Distinction in HIV.

Food Pantry

Our food pantry is a dedicated service exclusively for patients living with HIV who access our clinic, pharmacy, and behavioral health services. Guided by the belief that food is medicine, eligible patients are invited to shop once a month.

Behavioral + Mental Health

We believe that quality mental health programs go hand-in-hand with quality medical care. Together, these services give our patients the best opportunities to not only live healthy lives with HIV, but thrive. Whether it’s talking about depression or anxiety, managing psychiatric medication, or navigating life changes, we offer quality, inclusive care to all.

Case Management

We connect you to services and resources, and provide support throughout.

Prevention

We provide free, confidential HIV and Hep C testing, free condoms and lube, and help accessing PrEP.

HIV & Hep C Testing:
Monday–Friday (10:00pm – 4:00pm)

LifePoint

We provide free syringes, safer injection supplies, safe syringe disposal, Naloxone, and harm reduction training.

Syringe Exchange:
Monday–Friday (10:00pm – 4:00pm)

Legal Services

We help with legal documents, housing issues, estate planning documents, and discrimination.

    A group level intervention that focuses on holistic health, mental health, HIV education, spirituality, and substance use.

  • Positively Aging

    Positively Aging

    Second and Fourth Tuesday of the month (3:00pm – 4:30pm)

    Positively Aging is here to provide compassionate care designed specifically for adults aged 50 and older living with HIV. Whether you are newly diagnosed or have been managing HIV for years, we are committed to supporting your unique health, emotional, and psychosocial needs every step of the way.

  • The Tea Room

    The Tea Room

    Wednesday & Thursday (1:00pm – 5:00pm)

    The Tea Room, TPAN’s drop-in youth center, is a safe space for young adults to congregate and find help and community. Here you will find a washer-dryer for your use, snacks, TV, video games, a computer, and easy access to services, including medical care and case management.

    Anyone between the ages of 18-29 and a member of the LGBTQIA+ community or an ally is welcome, regardless of HIV-status, race, or gender.
